Transcripts

In order to request a transcript you will need to complete a transcript order form.  The form can be accessed by clicking on the following link:

If you are unable to print off the form you may submit a request through the mail.  Phone requests will not be accepted. Since transcripts contain confidential information, your signature is required for the transcript request to be processed.

Please include: 

1. Student Identification Number
2. Name used at U of M or previous names
3. Date of Birth
4. Your signature - required
5. Last term of attendance
6. Number of copies
7. Your current telephone number
8. Complete address of where Transcript will be sent

9. Your return mailing address

Payment Information:

$3.00 fee for an official Transcript

Unofficial copies are free (Unofficial transcripts can be viewed on CyberBear if you have attended since Summer 1990.)

Faxed copies are unofficial and cost $15 per fax 

Federal Express or Overnight Mail - $15, and $3 Transcript Fee
